The MSCT Appearance of Acute Pancreatic Trauma and Follow-up of Complications

Abstract: Objective To summarize the data on pancreatic trauma and related complications with MSCT imaging being performed in the follow-up review.Methods A retrospective analysis on MSCT examination data of 28 cases patients with clinical diagnosis of pancreatic trauma.With reference to the trauma surgery branch (AAST) proposed classification standard,pancreatic injuries were classified and the level of damage was observed in the pancreas as well as related complications and at follow-up review of MSCT manifestations.Results 3 cases of misdiagnosis were found on first diagnosis,25 cases had direct signs that led to the diagnosis of pancreatic injury on first assessment,28 cases of pancreatic trauma had visible indirect signs.Grade Ⅰ injury was found in 8 cases;grade Ⅱ injury was displayed in 6 cases,grade Ⅲ injury was found in 7 cases,grade Ⅳ injury was observed in 5 cases and grade V injury was diagnosed in 2 cases.Enhanced scan can clearly show the range of pancreatic damage and the laceration zone.Laceration zone of more than 50% displays a ruptured main pancreatic duct.Related complications included 9 cases of pseudocyst secondary to pancreatic injury,Pancreatic vascular injury in 6 cases,abdominal or peripancreatic infection in 2 cases and pancreatic atrophy in 2 cases.Conclusion MSCT can accurately display the pancreatic injury.Enhanced scan can significantly increase the positive rate of diagnosis of pancreatic injury,to more accurately assess the extent of damage to the pancreas and grading thereof.MSCT follow-up observed pancreatic recovery,pseudo cyst formation and the change of the size of the cysts and the abdominal cavity or peripancreatic shrinking.Pancreastic trauma complications such as infection provide the basis for clinical doctors to select reasonable treatment.
