Radiologic Hepatic Capsular Invasion on MSCT in Prediction of Microvascular Invasion of Hepatocellular Carcinoma

Abstract: Objective The purpose of this retrospective study was to clarify the diagnostic value of radiologic hepatic capsular invasion on multiphasic MSCT for predicting the presence of microvascular invasion (MVI) in hepatocellular carcinoma (HCC).Methods 132 patients with pathologically proved solitary hepatocellular carcinoma were retrospectively analyzed.All cases were performed on a multiphasic 64-MSCT scanner before operation.Radiologic evidences of hepatic capsular invasion were compared with surgical histopathology results.Results Radiologic hepatic capsular invasion was closely correlated with the presence of MVI and hepatic capsular invasion in histopathology.The presence of radiologic hepatic capsular invasion had a sensitivity of 83.3% (40/48),a specificity of 67.9% (57/84),and an accuracy rate of 73.5% (97/132) in predicting MVI.Conclusion The characteristics of hepatic capsular invasion on preoperative CT scans is a simple and effective noninvasive technique for detecting MVI.
