A Modified Surgical Posteromedial Approach to Posterior Pilon Fractures with Double-plating Fixation

Abstract: Objective To present a modified posteromedial approach for the treatment of posterior pilon fracture with double-plating fixation.Methods Eighteen patients(11 males plus 7females,average,44.5 years old)with posterior pilon fracture were retrospective analyzed via modified posteromedial approach with double-plating fixation from March to December in 2014.The fracture healing times were evaluated using radiological images.The ankle joint functions at last follow-up visit were evaluated by American Orthopaedic Foot and Ankle Society(AOFAS)ankle-hind foot score and visual analogue scale(VAS).Results All incisions healed within primary period without wound complications.Fifteen patients got a mean period of final follow-up was 14.7 months(12-19 months).All the patients achieved fracture union,and the mean healing time was 12.6 weeks(12-14 weeks).The average score of AOFAS ankle-hind foot was 82.5(70-87),and VAS score at rest,in motion and at weight bearing walking was 0.33,0.94 and 1.61 respectively.ConclusionModified posteromedial approach provides adequate exposure for the posteromedial and posterolateral fracture fragments of the posterior pilon fractures,the double plating internal fixation is simple and effective,meanwhile,the short-term outcomes are satisfied.
