Clinical and dermoscopic manifestations of acute telogen effluvium

Abstract: Objective: To investigate the clinical and dermoscopic features of acute telogen effluvium(ATE). Methods:Clinical manifestations and dermoscopic features of 15 patients with ATE were respectively analyzed, while ten healthy subjects served as normal controls. Results: All patients experienced acute and diffuse hair loss and had weakly positive hair pull tests. The duration of disease ranged from 5-11 weeks, with an average of 7.3 weeks. Compared with normal controls,the dermoscopic features of ATE included increased density of new vellus hairs, and progressive hypopigmentation of proximal part of the hair shaft. The proportion of hypopigmented hair shafts was negatively correlated with the disease course. The proximal part of the shed hair shaft was also hypopigmentation. Hair roots exhibited club hairs of telogen or columnar hairs of exogen. Hair roots became narrow during exogen. All subjects regained healthy hairs after recovery. Conclusion: Diagnosis of ATE can be made by the distinct set of clinical and dermoscopic features. The proportion of hypopigmented hair shafts and the density of newly grown vellus hairs could be used to predict the disease course. The disease is self-limited, and can be completely recovered within a few months.
