I don't know why this happens so often.
Japan's pilots need serious training in mountainous IFR operations. If they were preparing to make an IFR landing there is absolutely no reason this aircraft should have crashed, IFR approaches are strictly charted down to exact speeds and times, plus all modern aircraft have GPS onboard in some form or another.
This aircraft probably also had GPWS which means they either disabled it or ignored it. Never ignore GPWS if you can't see what's in front of you.
There is an old American Airlines video on youtube that has a short course on CFIT (controlled flight into terrain). Every aviator with internet access should watch it. Could save your life.
Sad accident.
