Effect of Radical Surgery and Radiotherapy on Local Control Rate, Survival Rate and Complications in Patients with Early Stage Glottic Carcinoma

Abstract: Objective To study the effect of radical surgery and radiotherapy on local control rate,survival rate and complications in paticnts with early stage glottic carcinoma.Methods 80 cases of patients with early stage glottic carcinoma were selected.They were divided into the radiotherapy group and the operation group,each group had 40 cases.The radiotherapy group was treated with radiotherapy combined with small dose chemotherapy,and the operation group was treated with operation combined with small dose chemotherapy.Compared the clinical efficacy,improvement of laryngeal function,complications and recurrence and metastasis between the 2 groups.Results Survival and disease-free survival and local control rates of the radiotherapy group were 95.00%,80.00% and 85.00%,and of the operation group were 92.50%,87.50% and 90.00%,the difference had no statistical significance(P > 0.05).Patients with laryngeal function completely improved and partial response in the radiotherapy group were more than the operation group,none response cases was less than the operation group,the difference had statistical significance(P < 0.05).In the surgery group,there had 1 case of laryngeal cavity stenosis,2 cases of subcutaneous emphysema,2 cases of pulmonary infection and 6 cases of incision infection,in the radiotherapy group,there had 3 cases of dysphagia,3 cases of pulmonary infection and 7 cases of radioactive dermatitis/pharyngitis,the difference in complications between the 2 groups had no statistical significance (P > 0.05).In the surgery group,there had 8 cases of recurrence,6 cases of primary tumor recurrence,cervical lymph node metastasis in 2 cases,in the radiotherapy group,there had 9 cases of recurrence,7 cases of primary tumor recurrence,2 cases of neck lymph node metastasis,the difference in recurrence and metastasis between the 2 groups had no statistical significance(P > 0.05).Conclusion Surgery and radiotherapy as adjuvant treatment to conventional chemotherapy for early stage glottic carcinoma have similar efficacy,complications and recurrence and metastasis.Adjuvant radiotherapy and chemotherapy has smaller influence on laryngeal function and with high quality of life.
