n for straightening the penis (Diiplay).—i. The penis has beenstraightened after a transverse incision through the skin, and contracted fibrous bands.Note the longitudinal defect. 2. The transverse defect sutured longitudinally, with on-end mattress sutures. The glandular portion of the urethra is shown completed. corpus spongiosum, the thickened envelope of the penis and of the con-tracted septum between the corpora cavernosa. The skin defect isclosed longitudinally or with a pedunculated flap, or is grafted (Figs.279 and 28c). In any operation in which the formation of the urethra, penile orperineal, is attempted, an external urethrotomy, or in selected casesa suprapubic cystostomy should be done, and a permanent catheterinserted. Of the numerous operative procedures advocated for the relief ofhypospadias, I shall describe a few of the best. Every surgeon will haveto adopt suitable modifications or combinations of the methods inorder to lit the particular case. 3i8 PLASTIC SURGERY
